REFERRAL SOURCES

Guidelines for Appropriate Referrals to LIFE Lutheran Services

If one or more of the following statements apply to your patient's situation, please contact LIFE Lutheran Services at 717-264-5433. Our Intake Coordinator can further determine eligibility. A doctor's order is not necessary for enrollment in the LIFE program.

• Medical care and supportive services are fragmented.
• There have been recent hospitalizations due to illness or injury, resulting in decreased function.
• Falls occur frequently.
• There exists a degenerative, long-term prognosis.
• There are needs not covered by Medicare or Medicaid that may be addressed through the LIFE comprehensive support system and prevent facility placement for those individuals who desire to remain in the community.
• The individual is socially isolated or living alone with diagnosed dementia.
• Medical appointments are being missed.
• The individual is involved in a day care program but missing attendance days due to medical fragility or other circumstances.
• There is difficulty with medication management.
• There is a history of multiple emergency room visits, hospitalizations, or short-term nursing home stays.
• There is an unstable or questionable caregiver situation.
• Benefits will not pay for the level of personal care that is needed.
• Case Manager must spend extensive time commitment to ensure health and safety.
