Guidelines for Appropriate Referrals to LIFE Lutheran Services
Medical care and supportive services are fragmented.
There have been recent hospitalizations due to illness or injury, resulting in decreased function.
Falls occur frequently.
There exists a degenerative, long-term prognosis that the existing service delivery system has been unable to address.
There are needs not covered by Medicare or Medicaid that may be addressed through the LIFE comprehensive support system and prevent facility placement for those individuals who desire to remain in the community.
Individual is socially isolated or living alone with diagnosed dementia.
Medical appointments are being missed.
Individual is involved in a day care program but missing attendance days due to medical fragility or other circumstances.
There is difficulty with medication management.
There is a history of multiple emergency room visits, hospitalizations, or short-term nursing home stays.
There is unstable or questionable caregiver situation, history of APS involvement.
Benefits will not pay for the level of personal care that is needed.
Case Manager must spend extensive time commitment to ensure health and safety.
