About LIFE

Services

At LIFE Lutheran Services you will have a team of experienced and caring health care professionals focusing on YOU. They will get to know you, your living situation and your preferences. They will work with you and your family to develop and update a plan of care for you that is based on your goals. Because LIFE is a provider sponsored health plan, your LIFE health care team works with you to make decisions about your health care. No higher authorities will overrule what you, your doctor and other care providers agree is best for you. If you disagree with the team about your care plan, you have the right to file an appeal.

LIFE services include all primary medical care, which will be provided at the LIFE Center, and specialty services, prescription drugs and all medically-necessary transportation. Based on your plan of care other services may include:

• Primary medical care and specialty services
• Day services and recreational therapies
• Laboratory and diagnostic services
• Vision, dental, hearing and foot care
• Prescription drugs and other medical supplies and equipment
• Nursing, Physical Therapy, Occupational Therapy and Speech Therapy
• Social services and behavioral health care
• Home health care and personal care
• Laboratory and diagnostic services
• Nursing home care, acute hospitalization and ambulance services
• 24/7 access to emergency/urgent care
• Nutritional counseling and meals
• Transportation
• Chaplain services
